Troubleshooting

• "I accidentally clicked 'Submit Form' and now realize I missed entering some of my data or found an error in what I submitted."
  Please call or email and we will update your responses.
   
• "I want to print a copy of my lab's results to keep for our records."
  On the last page of the survey, there will be a “Submit Form” button. This will create a summary of your responses for that specimen. You may print a copy of this summary for your records, or you may email a copy of the summary to an email address of your choosing using the link at the top of the summary page. 
   
• "How do I know if my data has been successfully submitted?"
  If, after you click the "Submit Form" button, you see a printable report that has all your responses in a summary table, your data has been successfully submitted.
