HMO Action Plans and Collaboration Plans
The Clearinghouse distributes recent copies of Collaboration Plans submitted by health maintenance organizations. Earlier copies of the Collaboration Plans and all Action Plans are now at the Minnesota Historical Society.
- Action Plans describe how health plan companies deliver their services, including information on credentialing standards, available physician and non-physician providers, provider capacity and access, quality improvement activities, service to high-risk and special-needs populations, and service to rural communities.
Minnesota Statute 62Q.746 requires that this information be made available if requested by the Minnesota Department of Health. In 2001 the Minnesota Legislature repealed Minnesota Statute 62Q.07 which required annual filing of Action Plans.

The Clearinghouse no longer distributes Action Plans.
- Collaboration Plans tell what health maintenance organizations will do to meet identified public health goals for communities in the areas they service. Local public health units and other community organizations in the health maintenance organization's service area provide input to the collaboration plan.
Minnesota Statute 62Q.075 requires that collaboration plans be filed with the Commissioner of Health and describes the filing requirements, the information to be included in the plan, and the details of the review process. The Minnesota Department of Health, Public Health Practice Section receives these plans.
The current collaboration plan for 2015 - 2019 is a combined plan compiled by the Minnesota Council of Health Plans (MCHP). Health plan members include Blue Cross and Blue Shield/Blue Plus of Minnesota BCBS, HealthPartners (HP), Medica, Metropolitan Health Plan (MHP), PreferredOne (P1), Sanford Health Plan of Minnesota, and UCare.
The 2015-2019 Collaboration Plan is organized in two ways. First, it describes ways health plans collaborate with local public health agencies. Second, it provides information about Minnesota health plans. The Collaboration Plan is updated regularly and is available on the MDH and MCHP websites.
Collaboration Plans are filed by health maintenance organizations every four years and updates are filed every other year. Prior to October 2004 the collaboration plans were filed every two years.
