May 2024 renewal announcement
Renewal notices for speech-language pathologists and audiologists with a May 31 expiration will be sent by email on April 1.
Minnesota Statutes, section 148.5191, subdivision 3 requires licensees to notify the Minnesota Department of Health (MDH) within 30 days when there is a change of name, address, or employment. A name change request must be accompanied by a copy of a marriage certificate or court order.
Submit the Change of Information Form (PDF) if your name, employment, address or email address has changed to ensure timely receipt of your renewal notice.
If you do not receive your renewal notice on April 1, or you would like to receive the renewal notice by mail, please email the Speech-Language Pathology and Audiology section at health.slpa@state.mn.us.
Reminder — DHS resumed background studies in 2022
The Department of Human Services (DHS) resumed processing background studies for MDH providers as of March 2, 2022. Any speech-language pathologists or audiologists who have not already completed a full fingerprint background study will need to complete one during their next renewal period or when submitting an initial or temporary license application.

If you have submitted an application and a background study, please be patient as we are reviewing applications as quickly as possible.
If you have already completed a full fingerprint background study, no action is needed from you at this time.
