If you have not registered your company information with this system, please click the "New User Registration" button. If you are currently registered with the system, please select the "Registered User Sign-on" button.

New User Signup page
Begin filling in the information fields on this page. The first name and last name fields should be the name of the person MDH can contact for issues regarding projects.
Fill in all remaining fields, including:
- your contractor license, which should be in this form: AC1234 (no space between the AC and the number)
- your company email address (this is where all notification and payment confirmation emails will be sent)
- Your password (this is set to MDH internet security standards. It must be 8 to 10 characters long, in mixed upper and lower case, number and special characters. Example: GoV1k1ng$!)

Registered User Login page
Enter the email you wish your electronic notification and electronic payment confirmations to go to. Then enter the password you created when you registered as a new user.
If you forgot your password, enter your email and then click the "Forgot Password" link. A temporary password will be emailed to you. Use this temporary password to enter your account and then change your password to one you want.
If you wish to change your password or edit your company's information, enter your email and password and then click the "Edit Profile/Password" link. You will now be able to edit your company's information and/or change your password.
If you change your password or edit your company information, make sure you click the "Save and Continue" button to move to the next page.

Notifications page
Clicking the "Enter New Notification" button at the top of this page allows you to begin entering a new notification and will take you to the "Project Information" page.
Underneath this button are two lists: un-submitted notifications and submitted notifications.
- The Un-submitted and Unpaid Notification list contains your notifications that have not been submitted or paid for yet. Click the "Edit" link to continue filling out your notification. You will have to click the "Save & Continue" button on the bottom of the pages until you get to the section of the notification where you left off.
- The Submitted Notification list contains your active notifications. They are arranged in order, with the most current "Start Date" at the top. You can print any notification on this page by clicking the "Print Notice" link, or you can amend any of these notifications by clicking the "Amend" link.
- NOTE Once a notification has passed the "End Date", it will drop off this list and cannot be amended further.

Project Information page
Enter the Project Contact first and last names in the required fields. Make sure you are entering your company's Responsible Person or your company's Project Manager in charge of the project. This information is important in case MDH needs to contact your company regarding the project.
Next select the necessary project details: work scope, project type and other information.
Finish this page by choosing an air monitor from the pull-down menu.
- If your project does not require air monitoring (such as an outdoor siding project), select <None>.
- If you are conducting all air monitoring (adjacent and clearance) and sending it to an analytical laboratory for analysis (or having a consultant come on-site and analyze the samples), select the name of the consultant or laboratory conducting the analysis and then check the "Air Sampling Analysis Only?" check box.
- If you are conducting adjacent monitoring and sending it to an analytical laboratory for analysis (or having a consultant come on-site and analyze the samples), but the clearance sampling is being conducted by a consultant, select the name of the consultant conducting the clearance from the pull down menu and then make sure the "Air Sampling Analysis Only?" check box is unchecked.
Click the "Save and Continue" button to move to the next page.

Building Owner Information page
Enter all the information for the building owner: Building owner name, address, telephone number, etc. Please do not use abbreviations for cities, i.e. MPLS for Minneapolis.
Click the "Save and Continue" button to move to the next page.

Building Information page
Select either "Residential" or "Non-residential" for the building/site you will be working at. If you select "Residential", the Building name will automatically default to "Residence". If you select "Non-residential" you will have to enter the name of the building/site you are working at.
If the building owners address is the same as the building you will be working at (like a homeowner living in their home) you may check the "Building Address Same As Building Owner?" check box. This will auto fill the building's address. If the building owners address is different, then you need to fill out the building owners address.
Make sure the rest of the information is filled out as well, including county, previous use, etc.
Click the "Save and Continue" button to move to the next page.

Project Details page
Fill in the project details on this page. Put in the amount of friable or non-friable material you will be removing, along with start and end dates and work shift times.
Next enter the company that did the asbestos inspection of the property, and what type of asbestos-containing material is being removed and where it is located.
If you are going to be onsite at the start of the project make sure the "Onsite?" check box is checked (it will default to this automatically). However, if you are not going to be on site at the start of the project (for instance, a maintenance project at the University of Minnesota) make sure the "Onsite?" check box is unchecked. Otherwise it appears that you are onsite conducting asbestos-related work. When you are going to be onsite, amend the original notification and check the "Onsite?" checkbox.
Finish the page by selecting the type of work practices you will be following.
Click the "Save and Continue" button to move to the next page.

Waste Transporter and Landfill page
On this page you select your waste transporter and landfill from the pull down menus. If you do not see your transporter and/or landfill on the list, contact MDH. We will enter the waste transporter and/or landfill into the database and it will be available for selection in 24 hours. In the meantime you can do one of two things:
- wait until the next day to finish entering your notification (it will be saved under the Un-submitted and Unpaid Notices list in your account), or
- select a different transporter and/or landfill from the list. The following day, amend this notification for the correct waste transporter and landfill.
Click the "Save and Continue" button to move to the next page.

Fee Calculation page
If you are conducting a residential project the fee will default to $35. If you are conducting a non-residential project, you will need to enter your total cost of abatement in the 2% Fee box. Then select if this fee is covering the air monitoring costs and if the project is a time and materials project. Finally click on the "Calculate" button. Your 2% fee is calculated for you.
Click the "Save and Continue" button to move to the next page.

Review Notice page
You can review your notice on this page. Check to make sure address numbers aren't transposed or that the direction of the street should be North instead of South, etc. If you find information you wish to change, click on the "Edit" link for that page. When you finish editing that page, click on the "Save & Continue" button on the bottom of the page. Keep clicking on the "Save and Continue" buttons until you arrive back at the Review Notice Page. If all the information is now satisfactory, scroll to the bottom of the page and click the "Save and Continue" button to move to the next page.

Notice Submission page
You are now ready to submit your notification. Please read the statements on this page. If you agree with the statements on this page and enter your first and last name. This does not have to match the project contact information or the responsible person information. Then after you enter your name, click the "I Agree" button. You will be taken to the bank portal.
If you do not agree with the statements, do not enter your first and last name and click the "I Disagree" button. You will be asked to mail your notification and payment into MDH.

Bank Portal page
Click on the bank logo in the middle of the page to be taken to the bank website to pay for your notification.

Pay page
This page is confirming that the amount you wish to pay for is correct. If it is, click on the "Continue" button to continue payment.
If it is incorrect, click "Cancel". You will have to return to your MDH account, amend the notice with the correct 2% fee, and then continue on to the bank pages to pay for the notification.

Select Payment Method page
On this page you will select your payment method. Choose one of the credit cards from the pull down menu, enter the required information and click the "Continue" button to move to the next page.
If you click the "Cancel" button, the transaction will be stopped.

Payee Information page
This page is verifying the information of the owner of the credit card. Make sure that cards owner information is correct and that the billing address for the card is correct. Click the "Continue" button to move to the next page.
If you click the "Cancel" button, the transaction will be stopped.

Verify Payment page
On this page, review all the payment and credit card information. If it is correct, enter the three digit code on the back of your credit card (by your signature) and click the "Confirm" button.
If you click the "Cancel" button, the transaction will be stopped.

Payment Confirmation page
This page confirms that you paid for your notification. Additionally, you should receive an email confirming that you paid for this notification.
IMPORTANT Be sure to click the "Continue" button to finish the notification process. If you do not click the "Continue" button, MDH will be unable to issue your permit.

Print Notification page
You may now print your notification by clicking the "Print Notice" button. Your notice will be generated as a pdf and once it is generated, you can print it. From this page you may enter another notice by clicking on the "Enter Another Notice" link which will take you back to the "Notifications" page, or you may click the "Logout" link to logout and end your session.

Printed notice
You can print your notice from this page by clicking the "Print Notice" button. The notice will pop-up in another window as a pdf. Select "File" on the pdf toolbar and then "Print" to print your notice, or click on the small printer icon on the pdf shortcut bar.
