Health care organizations that participate in the federal 340B Drug Pricing Program—known as Covered Entities—are required to annually report data related to their 340B prescription drug purchases and payments to the Minnesota Department of Health (MDH). MDH submits its findings in annual reports to the Legislature on the reported data each November.

NEW: MDH release final reporting guidance for the 2025 340B reporting cycle. Beginning in 2025, Covered Entities are required to report 340B administered drugs—drugs given to patients in an outpatient setting. To accommodate this change, this year's reporting deadline for Covered Entities is May 30. Guidance documents are available below in the Report Resources section.
Report Resources
To register for the first time, update an existing account, and/or report data, reporting entities should access the online reporting system, Rx Data Portal. Data submitters may visit the Rx Data Reporting Portal Help site for more resources.

- Form and Manner for 340B Covered Entities—April 2025 (PDF)
- Minnesota statues require submission by April 1 of each year. Thus, April 1 remains the deadline in the Form and Manner. Please note the deadline for 2025 is May 30.
