Recertification

Step 4 MDH Notification

MDH will complete its review of all information and the first recertification report is reviewed by the HCH Certification Committee. MDH HCH will keep you informed of this process through its progression, including the outcome and recommendation of the HCH Certification Committee.

The recertification letter will go to the Assistant Commissioner of Health for signature and the applicant will receive a copy of the recertification letter, the report, and instructions regarding next steps and payer notification supporting reimbursement for care coordination services.

Recertified organizations will be provided with a HCH Seal of Certification to display within the organization. An electronic version, which may be used in marketing and print materials, will also be provided.

Appeals: If an application for recertification is denied, the commissioner must notify the applicant in writing the reasons for denial. The applicant has 30 days from the date of receiving notice to appeal the decision.
