The Case for Weight Inclusive Healthcare

In this webinar, you'll learn about weight inclusive healthcare, including its principles, evidence, and benefits. We'll discuss how weight stigma in healthcare can harm both physical and mental health, causing people to avoid care, experience medical mistakes, develop eating disorders, and face other health issues. Health care providers and policy makers will discover why addressing weight stigma is important for providing trauma-informed and patient-centered care for everyone. You'll receive resources for further learning, tips on creating weight inclusive healthcare environments, and a better understanding of how to support patients in larger bodies.

Opportunities for Community Engagement in Health Care Systems

Fostering Belonging within the Workplace and within Health Systems

Fostering Belonging explores social identities and how our experiences of where we work, play and live influence our understandings and beliefs. In this webinar, we deepen our understanding of how biases toward social identities form and how they may show up in the workplace, especially in the areas of workplace culture and health care practices and policies. Belonging is introduced along with how to foster belonging in the workplace. Lastly, we discuss best practices of how Health Care Homes may continue to work towards providing more equitable and culturally sensitive care. Guiding Principles and Innovative Ideas for Engaging Patients and Families

Part II: Understanding Medicare and Medicaid Billing for CHW Services in MN

This is Part II of the MDH Health Care Homes sponsored webinar offered in June 2024. This webinar provides more detailed information on the billable Community Health Worker (CHW) services in MN as a result of the new Medicare Physician Fee Schedule Billing Codes for Community Health Integration and Principal Illness navigation. The webinar also held a Q and A segment that included representatives from both Minnesota Department of Health and Department of Human Services. It’s Not a Health Care Home Without Oral Health

The US surgeon general’s 2000 report declared that “oral health is integral to the general health and well-being of all Americans” and “must be included in the provision of health care and design of community programs”. The bad news: Many Minnesotans continue to suffer barriers to care resulting in persistent oral health disparities. The good news: Innovation makes integration possible. Panelists provide information, practical tools, and describe partnerships to integrate oral health into whole-person health homes that are equitable, accessible, and accountable. Presenters: Deborah Jacobi, RDH, MA, Policy Director, Apple Tree Dental; Jane Neubauer, Special Project Coordinator, Early Childhood Dental Network; Nancy Franke Wilson, Executive Director, Minnesota Oral Health Coalition.

- Describe the oral-systemic health connection and its implications for whole-person health.
- Utilize guidance in Minnesota’s State Oral Health Plan and medical-dental integration resources from the Minnesota Oral Health Coalition and the Early Childhood Dental Network.

The State of Colorectal Cancer in Minnesota
March 19, 2024
This webinar from the MDH Comprehensive Cancer Control Program (CCCP) highlights data on colorectal cancer incidence and mortality, and screening rates statewide and among populations experiencing the largest disparities for colorectal cancer. The CCCP team shares current guidelines and options for screening, and tools to support or improve colorectal cancer screening in clinics. Two primary care clinics also share ways that they have improved their screening rates.
Learning Objectives:
- Participants will be able to describe the populations more heavily burdened by colorectal cancer in Minnesota.
- Participants will be able to clearly outline to their patients the current screening guidelines and options for colorectal screening.
- Participants will be able to come up with 1-2 ways that they will enhance or implement changes in their clinic to improve colorectal cancer screening rates.

Care Coordinators: Bridging Care for Diabetes Management

Diabetes self-management is complicated, and supporting patients with diabetes can be challenging. Bridget Ideker from MDH explored these challenges and provided practice-based learning, tools, and resources to address them.

- Participants will be able to define diabetes distress.
