Health Care Homes Primary Care Stakeholder Group
Charter
The Minnesota Academy of Family Physicians, along with the Minnesota Department of Health’s Health Care Homes Program and Office of Rural Health and Primary Care, convened a group of primary care stakeholders in January 2020 focused on increasing investment in primary care in Minnesota. The stakeholder group, known as the Primary Care Stakeholder Group, agreed on the following purpose:
Increase investment in primary care services that are equitable, person centered, team based and community aligned, and will help achieve the goals of better health, better care, and lower costs.
The Primary Care Stakeholder Group collaborates in defining goals, objectives, strategies, and recommendations to increase investment in Minnesota primary care. The stakeholder group includes stakeholders from Minnesota health care professional associations, providers, payers, employers, consumers, and government agencies.
Objectives: To support the goal of investing in primary care for better health, better care and lower costs, the stakeholder group will:
- Engage stakeholders that represent all parts of the community and healthcare delivery system
- Work to strengthen the ability of primary care to improve population health
- Promote primary care workforce to ensure access to all:
- Support diverse workforce that reflects communities served
- Promote team-based care and adequate primary care workforce to address the needs of all Minnesotans
- Improve access in underserved areas
- Promote healthcare staff engagement, recruitment and retention
- Keep the patient at the center of care by supporting strategies that will:
- Support efforts to improve health and community outcomes:
- Empower consumers
- Increase coordination with community partners
- Integrate behavioral health services
- Allow clinicians to focus on the most appropriate outcomes by eliminating administrative burdens
- Promote investments in human assets, infrastructure and care integration that strengthens care delivery
- Support efforts to improve health and community outcomes:
- Implement primary care through value-based models that promote and align incentives across participants, including consumers
