Get Screened: SagePlus Heart Health Program

SagePlus is a free program that can help you improve your heart health. If you are eligible for the Sage Program, you can also participate in the SagePlus Program. 

Changing your lifestyle can go a long way to controlling high blood pressure and preventing heart disease and stroke. Improve your health by making informed and intentional lifestyle choices including:

  • Eating a heart-healthy diet with less salt.
  • Getting regular physical activity.
  • Maintaining a healthy weight.
  • Limiting the amount of alcohol you drink.

What SagePlus can do for you

When you sign up for the SagePlus program, you will be connected with one of our partner clinics. You will receive:

  • Heart health screening.
  • Health and wellness coaching.
  • Tools to help you make healthy changes.
  • Connections to resources and support.

Clinic staff will measure your:

  • Blood pressure.
  • Blood sugar.
  • Cholesterol.
  • Height and weight.

Clinic staff may ask about your health, diet, activity level, and other lifestyle behaviors.

A SagePlus health coach will help you understand what your measurements mean for your heart health, and together you will make a plan to eat healthier and become more active. If you smoke, your coach can help you make a plan to quit. You may have the option to join lifestyle change programs like nutrition education, walking programs, or Zumba classes.

Participants who attend the minimum number of sessions required AND have a follow-up screen may receive a grocery gift card.

Eligibility

People who are eligible for the Sage Breast and Cervical Cancer Screening Program are eligible for SagePlus.

  • Women (born female or transgender women and men) ages 35-64.
  • Uninsured and underinsured.
  • Income within program guidelines.

Supporting you in your path to health

Many things may impact your ability to be healthy, including where you live, your job, access to healthy and affordable foods, and more. Staff can refer you to services and benefits that can help improve your health like housing, food, transportation, or other resources.

  • Native American Community Clinic (NACC), Minneapolis
  • Neighborhood HealthSource Clinics
    • Central Clinic, Freemont Clinic and Sheridan Clinic, Minneapolis
    • North Metro Clinic, Coon Rapids
  • North Memorial Health Clinic, Brooklyn Center
  • Odam Medical Group, Crystal and St. Cloud
