Pediatric Care Coordination Community of Practice

Welcome to the Pediatric Care Coordination community of practice!  

The Minnesota statewide Pediatric Care Coordination community of practice provides a way for care coordinators to network, collaborate, share, and learn from each other. These activities build the capacity of systems that serve families of children and youth with special health needs.  The mnpedscare.com website is no longer active and Basecamp is our new communication platform.  

About the Community of Practice  

This community of practice is designed to facilitate communication and collaboration among the many people providing care coordination to a pediatric population across MN to better serve children and youth with special health needs.

Purpose  

Care coordination is an important function that provides optimal coordination and integration of services needed by the child and family. Relationships between the health care provider, the care coordination team, and the patient and family facilitate effective information sharing, goal setting, care planning and follow-up support.  

  • Care coordinators, care navigators, health coaches at primary care clinics  (e.g. family practice, pediatrics, med-peds)  
  • Care coordinators at specialty care clinics  
  • School nurses, public health nurses  
  • Case managers (e.g. county, non-profit, school)  
  • Social workers, waiver case managers  
  • Anyone providing care coordination activities and wanting to network with others!  

Join the community of practice

Joining the Pediatric Care Coordination community of practice gives you access to our online collaboration site hosted within Basecamp. You will be able to engage with peers, share knowledge, and access tools designed to support your work.

Members of the mnpedscare website will need to re-register for continued membership.

  • A confirmation email with information about Basecamp.
  • An invite directly from Basecamp to register your account and be connected to the Pediatric Care Coordination community of practice group.  

Learning opportunities  

As professionals who manage care for CYSHN connect, share, and learn from each other, they can improve child and family outcomes by building the capacity of all systems that serve families of CYSHN.

The CoP hosts discussions and quarterly webinars. Goals include:

  • increase knowledge of care coordination best practices;
  • increase skills needed for management of care; and
  • increase access to resources that are valuable for people who coordinate care for children and youth with special needs and their families.

Advisory group

The Pediatric Care Coordination community of practice steering committee is composed of representatives from state agencies; medical clinics and hospitals; counties and organizations across Minnesota. These leaders bring diverse perspectives and are committed to driving systemic change. Committee members serve for one-year terms and actively contribute to planning and decision-making for the community of practice.
