Incentives and Payments
MDH Health Care Homes (HCH) certification has benefits that can help sustain your practice. This includes care coordination services reimbursement, alignment with value-based payment and other alternative payment arrangements, and advantages for participants in the Centers for Medicare & Medicaid (CMS) Merit-based Incentive Payment System (MIPS).
Providing care coordination services can open the door to additional resources to help care for high-risk, high need patients. Minnesota Health Care Programs (MHCP) provides an opportunity for billable payments. Some commercial payers reimburse for care coordination services using the MHCP process and codes, and others reimburse through alternative payment arrangements or value-based programs.
