Information for Youth and Families
Child and Teen Checkups (C&TC is a benefit for children in Minnesota who are eligible for Medical Assistance (Medicaid).
C&TC includes well child checkups. The checkups help track growth and development, identify potential concerns, and develop a healthy relationship with your health care provider. As children age, the checkups help them take charge of their health, discuss health issues, and maintain healthy behaviors to support lifelong health.

- Head-to-toe exam.
- Time to ask questions and get answers about your child's health, behavior, and development and to talk about the child's learning, feelings, relationships, parenting and caregiver wellbeing.
- Write down any questions or concerns.
- Bring ANY forms you need filled out for Head Start, WIC, school, sports or child care.
