Child and Teen Checkups
The child and teen well-child checkups are a vital opportunity for healthcare providers to monitor a young patient's growth, development, and overall health. These visits offer a chance for providers to: Engage with families, answer questions, and provide guidance on lifestyle choices Help children and youth thrive Set a strong foundation for lifelong health and wellbeing.

Periodicity schedule and documentation forms
The C&TC Schedule of Age-Related Screening Standards, also known as the Periodicity Schedule, summarizes C&TC screening component requirements and recommendations by age. The required components must be completed, or screening exceptions documented, in order to bill for the enhanced C&TC bundled rate.
C&TC Visit Documentation Forms are age-specific forms which include appropriate C&TC screening components that include appropriate C&TC screening components.
Fact sheets for primary care providers
The C&TC fact sheets provide component-specific information on what is required or recommended, which staff may provide the service, procedure-specific guidance, how it must be documented, and resources for referral and follow-up.
Coordinators
Every county, three tribes, and some Integrated Healt Partnerships (IHP) have C&TC Coordinators who provide local support. Coordinators provide: Minnesota C&TC County/Tribe Coordinator List or contact DHS for IHP contacts at dhs.childteencheckups@state.mn.us.
