Speech-Language Pathologist and Audiologist License Fees
Submit the license fee listed below appropriate for your license period.
A one-time processing fee to cover MDH costs associated with a Department of Human Services (DHS) Applicant Background Study (PDF) has been required of applicants since Oct. 1, 2018. This study is different from any background checks conducted by employers. The MDH processing fee is separate from the fee paid to DHS to perform the background check.
Applicants renewing a license obtained before Oct.1, 2018, that expires after Jan.1, 2020, must also pay the one-time processing fee (MN Statute 148.5194, Subd. 7a).
Submit the fee listed below appropriate for your temporary license period.
Speech-Language Pathologist:
9 Months = $75 + $10.50 = $85.50*
12 Months = $100 + $10.50 = $110.50*
18 Months = $150 + $10.50 = $160.50*
Criminal Background Check Processing Fee:
*A one-time processing fee of $10.50 is collected by MDH to cover costs associated with a Department of Human Services (DHS) criminal background check. Background check fees paid to DHS are additional.
Audiologist:
9 Months = $191 + $13 = $204*
12 Months = $255 + $13 = $268*
18 Months = $383 + $13 = $396*
Criminal Background Check Processing Fee:
*A one-time processing fee of $13 is collected by MDH to cover costs associated with a Department of Human Services (DHS) criminal background check. Background check fees paid to DHS are additional.
A $30 fee will be assessed to Non Sufficient Funds payments. Applicant must resubmit with certified funds.
Find the month the license application will be received in our office to determine the license fee amount. If you have already submitted a Department of Human Services Applicant Background Study (PDF) for your licensing prior to Oct. 1, 2018, you do not need to pay MDH a required one-time background processing fee until your renewal, after Jan. 1, 2020, per MN Statute 148.5194, Subd. 7a.

- Temporary License Fee for Applicants Credentialed by ASHA, ABA or other states: ($25 + $10.50 Surcharge) $35.50
- Verification to Other States: $25
