Swing Bed Hospital - MDS Effective July 1, 2002
The purpose of this Minnesota Department of Health (MDH) Information Bulletin is to notify Medicare-certified Swing Bed Hospitals that, starting July 1, 2002, Medicare beneficiaries receiving Skilled Nursing Facility (SNF) level care in swing bed hospitals will be phased into the SNF Prospective Payment System (PPS) and Swing Bed Units will have to perform Minimum Data Set (MDS) Assessments on all Swing Bed patients. The new form is named Minimum Data Set (MDS) For Swing Bed Hospitals (SB-MDS).
Until now, swing bed providers have not performed MDS assessments. Effective July 1, 2002, swing bed providers will have to complete and transmit to the Centers for Medicare and Medicaid (CMS) SB-MDS assessments on all patients who meet Medicare Part A Services eligibility requirements for a certified-hospital swing bed. The new Assessment Form to be utilized for this MDS assessment, Minimum Data Set (MDS) for Swing Bed Hospitals, is a subset of the actual MDS Assessment form utilized in nursing homes. It is two (2) pages long and contains approximately 45 items to be assessed. A Registered Nurse (RN) must complete and/or coordinate all MDS assessments.

Transition into the SNF PPS will be by hospital cost reporting year, and the first group of swing bed hospitals will transition into the SNF PPS for services furnished on and after July 1, 2002. (Please note this information was published on page 38128 of the Federal Register Vol. 67, No. 105/Friday May 31, 2002/Notices.)
Medicare Part A Services Swing Bed Eligibility Requirements are:
- Medically necessary 3-day hospital stay.
- Receipt of Part A care within 30 days of qualifying hospital stay.
- Part A care needed for a condition which was treated during the qualifying hospital stay.
- Receives a skilled level of care either skilled nursing or rehabilitation.
Resources For Learning How To Complete And Transmit The Swing Bed-MDS Form
The Minnesota Department of Health (MDH) is providing training on the completion and transmission of the SB-MDS by making available to Swing Bed Hospitals three videotapes and responding to questions via the MDH MDS Clinical Help Phone line. The training videos were produced by CMS which cover completion of the assessment form and the transmission of data to the CMS database. Included with this Information Bulletin are instructions for ordering these training videos:
- MDS Coding Video: A detailed item-by-item review of the two-page MDS form for Swing Hospitals (SB-MDS). Carol Job, R.N, of Myers and Stauffer, on two videotapes (Part I and Part II), will walk you through the SB-MDS and discuss the requirements for completing and submitting assessments and/or discharge/reentry tracking forms. Viewing both Part I and Part II will take 2.5 hours.
At the preparation of this MDH Information Bulletin, a Draft of the Swing Bed Minimum Data Set Assessment Training Manual is available from MDH for $12.00. It is recommended that this document be used when viewing the CMS MDS coding video. When the training manual becomes available on the CMS website, MDH will make a change directly to the MDH website with the CMS website address and MDH will no longer sell this draft manual.
- Swing Bed Transmission Video and RAVEN SB-MDS Video: Jack Williams of CMS will explain the transmission requirements and procedures that will be used by Swing Bed providers. John Jackson of FU Associates will review the new data entry software, emphasizing the areas where the RAVEN software has been modified for Swing Bed use. This videotape will take 1.5 hours to view.
