Training Opportunity: Improving Event Investigation through Root Cause Analysis (RCA)
Purpose:
The purpose of this information bulletin is to inform providers that MDH will sponsor three day-long trainings on root cause analysis (RCA) as a tool for investigating and preventing recurrence of serious events that can harm residents. This is designed as an introductory training for those who are new to the RCA process. These trainings will take place on:

Trainings will run from 8:30 – 3:30 each day, and a light breakfast and lunch will be served.
Content will be the same on each day; please register for just one session. This session is appropriate for administrators, DONs, staff development, and any staff/departments involved in safety and quality improvement in the organization at any level. As we anticipate demand will be high, space is limited to a total of three (3) participants from each facility. There is no charge to participate.

An agenda will be placed on this information bulletin site no later than the Friday, June 25th, 2010. Monitor this site for this information. Training materials will be provided on the day of training.
Registration
An online registration link has been placed on the no charge registration web site.
Providers must register online to participate in a training session. Space is limited to three participants from each facility.
