What to report

SARS-CoV-2 infection (COVID-19) (unusual case incidence, critical illness, or laboratory confirmed cases) must be reported to MDH within one working day.

How to report COVID-19/SARS-CoV-2

Case report forms for health care (e.g. health care providers, clinics):

  • COVID-19 Case Report Form (PDF)
    This individual case report form can be used to report cases or deaths and is submitted to MDH by FAX at 1-800-233-1817. Updated 1/21/22
  • COVID-19 Patient Reporting Form
    This individual case report form can be used to report cases or deaths and is submitted to MDH electronically.
  • COVID-19 Provider Portal
    The provider portal allows for submission of multiple case reports using spreadsheets or flat files.
  • Phone
    Any reportable infectious disease may be reported by phone to 651-201-5414 or 877-676-5414.

Report forms for deaths:

Deaths from COVID-19 are reportable to MDH.

Report form for hospitals:

Hospitals are required to submit reports on hospitalizations for patients who had a positive viral test for SARS-CoV-2 in the 14 days prior to admission or during their hospital stay.

  • COVID-19 Provider Portal
    The provider portal allows for submission of hospitalized cases using spreadsheets or flat files. Please choose the option for uploading COVID-19 hospitalization information via a csv or excel file.

Reporting for community settings:

Certain community settings (long-term care, K-12 schools, child care, corrections facilities, and shelters) may be required to follow specific reporting protocol and report COVID-19 to MDH.

Submitting clinical materials

For SARS-CoV-2 infection (COVID-19) (unusual case incidence, critical illness, or laboratory confirmed cases), submission of clinical materials (isolate, if available) to MDH is required.

Who is required to report

  • Health care practitioners (health care facilities, medical laboratories, and in certain circumstances veterinarians and veterinary medical laboratories) are required to report disease to the Minnesota Department of Health (MDH) under Minnesota state law.
    • Unless previously reported, every licensed health care provider who provides care to any patient who has, is suspected of having, or has died from a reportable disease is required to report.
  • Any person in charge of any institution, school, child care facility, or camp is also required to report disease to MDH.

  • Reporting of COVID-19/SARS-CoV-2 under the Minnesota Communicable Disease Rules, Chapter 4605.7080
    COVID-19/SARS-CoV-2 was previously reportable under part 7080 of the Minnesota Communicable Disease Rules. This changed with the 2024 rule amendments.
