What to report
SARS-CoV-2 infection (COVID-19) (unusual case incidence, critical illness, or laboratory confirmed cases) must be reported to MDH within one working day.
How to report COVID-19/SARS-CoV-2
Case report forms for health care (e.g. health care providers, clinics):

Report form for hospitals:
Hospitals are required to submit reports on hospitalizations for patients who had a positive viral test for SARS-CoV-2 in the 14 days prior to admission or during their hospital stay.

The provider portal allows for submission of hospitalized cases using spreadsheets or flat files. Please choose the option for uploading COVID-19 hospitalization information via a csv or excel file.
Reporting for community settings:
Certain community settings (long-term care, K-12 schools, child care, corrections facilities, and shelters) may be required to follow specific reporting protocol and report COVID-19 to MDH.

For SARS-CoV-2 infection (COVID-19) (unusual case incidence, critical illness, or laboratory confirmed cases), submission of clinical materials (isolate, if available) to MDH is required.

Who is required to report
- Health care practitioners (health care facilities, medical laboratories, and in certain circumstances veterinarians and veterinary medical laboratories) are required to report disease to the Minnesota Department of Health (MDH) under Minnesota state law.
- Unless previously reported, every licensed health care provider who provides care to any patient who has, is suspected of having, or has died from a reportable disease is required to report.
- Any person in charge of any institution, school, child care facility, or camp is also required to report disease to MDH.
