  EvaluationWeb is an on-line data collection and reporting system specifically for HIV testing and prevention activities.
  
  
• E-Authentication
  Before EvaluationWeb access is granted, all persons requesting access must be identity proofed via the CDC's Secure Access Management Services (SAMS) system. Once identity proofing is complete, Luther Consulting will provide instructions on accessing EvaluationWeb.
