The annual Learning Days conference brings together more than 200 health care providers, public health professionals and community partners to exchange information, advance health equity, and build relationships.
The 2026 Learning Days conference will be held Wednesday May 13 at the Heritage Center of Brooklyn Center.

The Health Care Homes Program is now accepting presentation proposal submissions for Health Care Homes Learning Days 2026, to be held on Wednesday, May 13, 2026, at the Heritage Center of Brooklyn Center.
Health Care Homes Learning Days is a statewide conference designed to support learning and collaboration among healthcare professionals, community partners, and organizations working to improve patient and population health. The conference focuses on practical strategies that strengthen patient and family engagement, care coordination, integrated care teams, and equitable approaches to care delivery.
We are seeking proposals for one hour breakout sessions that share effective practices, innovative tools, and real-world experiences that participants can apply in their work.

Health Care Homes Learning Days is an annual, in-person conference hosted by the Minnesota Department of Health. The conference brings together primary care teams, clinic leaders, community partners, and system stakeholders to share practical strategies that strengthen care coordination, advance health equity, and support integrated, patient-centered care across Minnesota.

Sponsorship opportunities provide targeted visibility and meaningful engagement while supporting access, equity, and the long-term sustainability of Health Care Homes Learning Days. All sponsors will be recognized in the conference app and may provide a logo, brief description, and link to their organization’s website for attendee access. Each sponsoring organization will receive a dedicated link to submit this information for inclusion in the app.

Health Care Homes Learning Days brings together clinic-based care teams from across Minnesota to learn, share, and strengthen coordinated care practices.
What to expect:
This event is designed for clinic nurses, care coordinators, and leaders working to improve patient-centered care delivery.
Contact Hours Credit
This activity has been awarded 6.0 contact hours.
It is the responsibility of each participant to determine whether these contact hours meet the requirements of their individual licensing board, certification body, or employer, and to submit documentation for acceptance as needed.
