Learning Collaborative
Learning Days Event

The annual Learning Days conference brings together more than 200 health care providers, public health professionals and community partners to exchange information, advance health equity, and build relationships.

This year's conference will be held May 14 at the Heritage Center: 
6155 Earle Brown Drive 
Brooklyn Center, Minnesota 55430.

Please join us for this annual spring event where Health Care Homes, public health, and community partners come together to share best practices, learn from each other, and form collaborative relationships.

The EventMobi Event app is available for both iOS and Android devices.

Search for "EventMobi Event" in the Apple App Store or Google Play Store to download.

Once installed, you will need to enter our event code: learningdays2025. Use the email address under which you registered for the conference to log in.

The Learning Days brochure features detailed session descriptions and speaker information, along with valuable networking opportunities.

The proposal submission deadline has expired. Presenters will be notified of their selection status by mid-March.

Connect with 200-plus primary care clinic providers and partners who are working together to improve the quality, outcomes and value of care while advancing health equity in their clinics and communities. Invest in the future of primary care by becoming a sponsor or exhibitor at Learning Days 2025. Please review the various options below; click on the links to register. Questions? Exhibitor: $300 – Registration includes company name in online program, table in exhibit hall, acknowledgement during conference, and one complimentary conference registration.

Silver sponsorship: $750 – Registration includes company name in online program, company name and logo with website on Learning Days webpage, table in exhibit hall, acknowledgement during conference, and one complimentary conference registration.

Gold sponsorship: $1,500 – Registration includes company name in online program, company name and logo with website on Learning Days webpage, table in exhibit hall, acknowledgement during conference, two complimentary conference registrations, and access to electronic list of conference attendees.

Your registration includes:

  • Full-day conference access on May 14 at the Heritage Center of Brooklyn Center

  • Up to seven continuing education credits (for health care professionals)

For attendees requiring overnight accommodations, a special block of rooms are secured at the Embassy Suites Minneapolis North, conveniently attached to the Heritage Center of Brooklyn Center. Rooms are available at the discounted rate of $139 per night plus tax.

To secure this special rate, reservations must be made by April 13, 2025. When booking, please mention the "Health Care Homes Learning Days" room-block or use this Available Rooms - Embassy Suites by Hilton Minneapolis North link. 

Limited rooms are available at this rate, so booking early is recommended.

There is plenty of free parking available.
