Changing the Accreditation Status of a Medicare-certified Home Health Agency
A Medicare-certified home care agency converting to deemed status or dropping deemed status must provide the following:
- Health Facility Identification Number (HFID)
- CMS Certification Number (CCN)
- Name of provider
- Type of change
- Date of change
- Notice will be signed by home health agency official
CMS-approved Accrediting Organizations (PDF)
If dropping deemed status, MDH will require the above letter and a letter from the accrediting organization that will establish the date of the change.
