Prescribed Pediatric Extended Care (PPEC) Center Licensure

1. Submit a completed application for licensure with supporting documents. 
2. Pay the required application fee. Fees are nonrefundable. 
3. Meet all applicable requirements under Minnesota Statutes, chapter 144H and related PPEC center rules. 
4. Mail the license application, supporting documents and fee payment to:

Federal Licensing, Certification and Registration section

St. Paul, MN 55164-0900 

5. Complete and submit a Construction Plan Submittal Package, as outlined in the Engineering Construction Plan Submittal Process for Healthcare Facilities. 
6. MDH completes required reviews and obtains clearances from Engineering Services, the State Fire Marshal and Health Licensing Evaluators. 

Note: A PPEC center license is issued when MDH determines that all licensure requirements have been met. An issued license is valid for two years.

1. Submit a renewal application at least 30 days before the license expiration date. 
2. Pay the required renewal fee. Fees are nonrefundable. 
3. Demonstrate that the licensee has provided basic services at the PPEC center within the past two years. 
4. Provide evidence that the applicant meets the requirements for licensure. 
5. Submit any additional information requested by MDH. 
6. Mail the license application, supporting documents and fee payment to:
