Birth Facility Performance Reports

The one-page Birth Quality Report summarizes the timeliness and completeness of selected birth registration items reported by a facility. Aggregate data for all birthing facilities in Minnesota is presented for comparison.

Facilities are listed alphabetically, so Abbott Northwestern Hospital’s report will appear when you open the files. Click on the hospital name and look for the small drop-down menu indicator just to the right of the name. If you can’t see the drop-down menu, download the file to your computer or enable editing for the file.  

Scroll to find the name of your facility, then click the report tab at the bottom of the screen to view the custom report for your facility.

Birth record filing timeliness

Minnesota law requires birth facilities to register births within five days. Has your facility met and kept the “timeliness in registration” goal? See data by facility, for 2020 to 2025: Birth Record Filing Timeliness by Facility and Year (Excel).

Minnesota Registration and Certification (MR&C) reports

MR&C has several reports available to help birth facilities track their performance. In MR&C, choose the Birth module, and click “Reports” in the Tasks menu at the left. Select the name of the report. Enter the desired date range (up to 365 days), select your facility name, and click “Generate report.”

Chronological birth log and fetal death log: Lists all births or fetal deaths entered in MR&C during a specific time period. Each line indicates a birth record and lists the state file number, date of birth, mother’s name, baby’s name, and father’s last name. These are the only reports that contain patient names. The report also shows if paternity documents were indicated on the birth record, the filing date, and the username of the birth registrar.  

Maternity and newborn activity: Data from births registered in MR&C for a specific time period. The report compares the number and percentage of births for each data field as reported by the hospital, alongside statewide numbers for comparison. Hospitals can use this report to identify potential reporting errors.  

Number and type of deliveries by attendant for a specific time period.  

Number of unknowns on birth records: The number of times and percentage of births for which specific birth data are marked “unknown” during a time period. For comparison, the report will also show statewide numbers and percentages.  

Time between birth date and date of filing: Shows the number of records filed during a given date range along with the minimum, maximum, and average number of days to file a birth record.
