Health and well-being for all: Accelerating learning about social determinants

CDC Foundation

This "meeting in a box," Health and well-being for all: Accelerating learning about social determinants, provides interactive materials you can use to discuss the factors that affect health, and for conversations about how to take action through teamwork and cross-sector collaboration.

Health and Well-Being for All contains interactive meeting materials you can use to discuss the factors that affect overall health, and to consider how to take action. This tool promotes teamwork and cross-sector collaboration to address social determinants of health.

Meeting participants will:

  • Discuss the determinants of health through a facilitated discussion and prompts: includes socioeconomic, behavioral, environmental, and other factors
  • Compare and contrast the internal and external issues faced by key stakeholders through a role-play exercise
  • Identify strategies for moving from group alignment to cooperative action through collective brainstorming

The resource includes a facilitator’s guide with three separate modules for discussion (asthma, gang violence, and obesity). It also includes role playing scenarios, a vibrant infographic map (3’x5’ durable fabric poster), and a carrying case.

When to use

Use this resource for staff meetings, community coalition meetings, or as a teaching tool for students pursuing public health as a profession.

Things to consider

You can purchase the Meeting-in-a-Box format of Health and Well-Being for All ($249), or download individual components at no cost.
