  • Minnesota Department of Health (MDH): Long COVID: A Post-COVID Condition
    Provides an overview of what we know about long COVID, what MDH is doing to understand its impacts, along with resources and support for people with long COVID, their caregivers, schools, workplaces, and guidance for health care providers.
  • Centers for Disease Control and Prevention (CDC): Long COVID Basics
    Provides detailed information on the range of long COVID or post-COVID conditions including those associated with hospitalization and intensive care.

Resources for Grantees

  • Grantee Reference Guide (PDF)
    This guide is for MDH Long COVID Network & Capacity Building Grants grantees. It contains topics related to the State Fiscal Year 2024-2027 grant period, including program, financial, and procedural requirements that govern the grant process. This document is subject to change. Always refer to this online version for the most up-to-date policies and procedures.
  • Incentive Guidelines for MDH Grantees (PDF)
    Provides details about grantee roles and responsibilities and the differences between incentives and stipends.
  • Invoice Form (PDF)
    Grantees must submit invoices using this standard invoice form. Please submit your invoice electronically as an e-mail attachment to the Long COVID Program inbox, health.longCOVID@state.mn.us.
  • Minnesota Supplier Portal  
    A central hub for grantees to log in and view payment details, as well as update address and contact information related to your organization. Vendor Assistance Help Desk: 651.201.8100, option 1. Follow the prompts after that.

    • Q: Can grantees invoice monthly?
      A: Yes. Grantees should send an email to their grant manager when invoicing monthly.
    • Q: Do we need to upload invoices to the supplier portal/SWIFT?  
      A: No, you can just send them to your grant manager.
    • Q: If our organization has multiple grants with MDH, is it possible to be paid in one lump sum?  
      A: Yes, this is often how it’s done. If you need help separating the information, we can assist with looking up reference numbers and amounts.
    • Q: Is there a template for timesheet tracking? What level of detail are you looking for? What do we need to provide for supporting documentation?  
      A: Start with what your organization is currently using for tracking staff time and follow your organization’s procedures. Some grantees include a PDF from their time tracking system when they submit their invoices. If you don't have a computerized system, you can send a photocopy of what you use.
    • Q: For budget revisions, does “less than 10%” refer to the line item, or the entire budget?  
      A: 10% refers to the line item.
    • Q: Can our organization’s address on the invoice form (in the "remit to" field) go to the address in the grant agreement?
      A: When you applied to be a state vendor/supplier ID, you provided a remit address. That's where payments will go.  
    • Q: Do we get checks or e-payments?
      A: You indicated this in SWIFT when you set up your supplier ID. If you entered your bank information when you set up your SWIFT account, then you will get an e-payment.
  • Photograph, Video and Audio Release Form (PDF)
    All photographs, videos, and audio used in MDH materials for external or internal distribution – including online, print and presentations – must be legally used.

  • Statute 
    Minnesota’s Long COVID Program was established by the Minnesota Legislature in 2023 (Minn. Stat. §145.361). The program, housed at the Minnesota Department of Health (MDH), serves to monitor and address the impacts of long COVID and related conditions across the state.
