Minnesota Department of Health Disease Report Card 
("Yellow Card")

The MDH Disease Report Card ("Yellow Card") can be used to report cases of most reportable infectious diseases in Minnesota residents to the Minnesota Department of Health.

  • Please ensure you are using the appropriate method of reporting disease.
    Not all reportable diseases should be reported to MDH on the yellow card.

  • Candida auris (Minn. Rules 4605.7080)
  • Carbapenem-resistant Enterobacteriaceae (CRE)
  • Cat scratch disease (infection caused by Bartonella species)
  • Coccidioidomycosis
  • Cronobacter (Enterobacter) sakazakii
    • There is also a supplemental reporting form for cronobacter sakazakii .
  • Dengue virus infection
  • Encephalitis (caused by viral agents)
  • Hantavirus infection
  • Hepatitis (all primary viral types including A, B, C, D, and E)
  • Histoplasmosis (Histoplasma capsulatum)
  • Influenza
  • Kawasaki disease
  • Kingella spp.
  • Leprosy (Hansen’s disease) (Mycobacterium leprae)
  • Leptospirosis (Leptospira interrogans)
  • Meningitis (caused by viral agents)
  • Psittacosis (Chlamydophila psittaci)
  • Retrovirus infection
  • Tetanus (Clostridium tetani)
  • Transmissible spongiform encephalopathy
  • Tuberculosis (Mycobacterium tuberculosis complex)
    • MDH requests that tuberculosis cases be reported by phone or fax (i.e., not by mail) to help ensure timely public health follow-up measures.
  • Typhus (Rickettsia spp.)
  • Zoster (shingles)
     
  • Sentinel surveillance for:
    • (Invasive) Candidiasis
    • Reporting carbapenem-resistant Acinetobacter baumannii (CRAB)
    • Clostridium difficile
    • Nontuberculous Mycobacteria, Pulmonary (NTM)
      Extrapulmonary Nontuberculous Mycobacteria (ENTM)
    • Respiratory syncytial virus (RSV)
