Minnesota Department of Health Disease Report Card
("Yellow Card")
The MDH Disease Report Card ("Yellow Card") can be used to report cases of most reportable infectious diseases in Minnesota residents to the Minnesota Department of Health.

- Candida auris (Minn. Rules 4605.7080)
- Carbapenem-resistant Enterobacteriaceae (CRE)
- Cat scratch disease (infection caused by Bartonella species)
- Coccidioidomycosis
- Cronobacter (Enterobacter) sakazakii
- There is also a supplemental reporting form for cronobacter sakazakii .
- Dengue virus infection
- Encephalitis (caused by viral agents)
- Hantavirus infection
- Hepatitis (all primary viral types including A, B, C, D, and E)
- Histoplasmosis (Histoplasma capsulatum)
- Influenza
- Kawasaki disease
- Kingella spp.
- Leprosy (Hansen’s disease) (Mycobacterium leprae)
- Leptospirosis (Leptospira interrogans)
- Meningitis (caused by viral agents)
- Psittacosis (Chlamydophila psittaci)
- Retrovirus infection
- Tetanus (Clostridium tetani)
- Transmissible spongiform encephalopathy

- MDH requests that tuberculosis cases be reported by phone or fax (i.e., not by mail) to help ensure timely public health follow-up measures.

- (Invasive) Candidiasis
- Carbapenem-resistant Acinetobacter spp. (CRA)
- Pseudomonas aeruginosa (CR-PA)
- Clostridium difficile
- Respiratory syncytial virus (RSV)
- Severe acute respiratory illness (SARI)
