MED-X Case Reporting

There are three methods of case reporting and each Medical Examiner (ME) office can determine which works best for you.

  1. ME site fills out Case Report Form and sends to MDH
  2. ME site notifies MDH of cases and MDH will fill out Case Report Form
  3. MDH conducts case identification at ME site

All notifications to MDH should be made at least on a monthly basis.

Case Report Form

  • MED-X Case Report Form (PDF)
    A MED-X Case Report Form will be used for each case. This form may be filled out by the ME, or by a MDH MED-X Program Investigator using any of the methods below.Updated 4/2020

1. ME site fills out Case Report Form and sends to MDH

The ME fills out the MED-X Case Report Form and sends to MDH. You may use any of the following methods to report a case.

  • Fax: 1-800-233-1817
  • Mail: MED-X Program

    PO Box 64975
    St. Paul, MN 55164-0975
  • 2. ME site notifies MDH of cases and MDH will fill out Case Report Form

    For individual case reporting, the ME office notifies MDH of each case meeting the case definition. You may use any of the following methods to report a case.

    • Phone: 651-201-5414 or toll free 1-877-676-5414
      If you are reporting by phone, MDH staff will fill out the MED-X Case Report Form for you.

    3. MDH conducts case identification at ME site

    For monthly reporting, the ME office would need to grant MDH staff access to your ME database or case files to obtain a case list on at least a monthly basis.

    A MDH MED-X Program Investigator will come to you and fill out the MED-X Case Report Form.
