Adverse Health Events Reporting

If you are looking for information about adverse health care events at a specific hospital, community behavioral health hospitals, or outpatient surgery center, use the drop-down menus below to select the reporting year and then select the facility name. Then click "submit" to see the report for that facility. If you can't find the name of the facility you are looking for in the list for a given reporting year, the facility did not report any adverse health events in that year.

It's important to remember that these events are very rare. Use this information to help identify questions or concerns and talk to your health care provider. Ask doctors and nurses about the steps they are taking to ensure the safety of your care.
