Connect a Family with WIC
Use the Minnesota WIC Referral Form to:
- Refer your patients to the Minnesota WIC Program.
- Send height, weight, and hemoglobin measurements to their WIC clinic.
- The WIC Hemoglobin schedule (PDF) outlines hemoglobin screening schedules for WIC and Child and Teen Checkups.
Request a Medical Formula
Minnesota WIC requires documentation of a qualifying medical condition(s) to provide medical formula and food for infants, contract formula for children over age one, or medical food for women.
- Step 1: Find a list of medical formula provided by Minnesota WIC.
- Step 2: Complete the Medical Formula Documentation (PDF)
- Step 3: Send to appropriate WIC clinic. Refer to Minnesota WIC Agency Directory.

About WIC
- Participant must be income eligible:
- Participants must be women, infants or children:
- Pregnant women.
- Breastfeeding women up to 1 year postpartum.
- Postpartum women up to 6 months postpartum.
- Infants up to 1 year old.
- Children 1 year old up to 5 years old.
- WIC participants must live in Minnesota to receive MN WIC benefits.
- WIC participants do NOT needs to be a U.S. citizen.
- WIC participants usually receive benefits in the county where they live.
- Applicants can:
- Apply for WIC using the Minnesota WIC online application.
- Use the Minnesota WIC Agency Directory to search for an agency near where they live.

- Medical Formula Documentation Form (PDF)
- The WIC Program requires a medical diagnosis to provide exempt infant formula, medical foods, and contract formula for children ≥1 year. This form can be used to submit the request for WIC approval.
- Minnesota WIC Referral Form
- Please complete this online form to refer your patients to the Minnesota WIC Program. This form may also be used to provide medical data to the WIC Program for current participants.
