Immunizations and Communicable Disease Reporting

Resources to help school health services personnel implement the Minnesota school immunization law, report diseases, and provide information to students and families.

  • Communicable Disease Reporting

    Schools are required to report vaccine-preventable diseases and other infectious disease outbreaks.

  • Immunization Requirements for School

    Minnesota Statute 121A.15 requires school-age children to be vaccinated against certain vaccine-preventable diseases or have a letter of exemption on file with the school. Schools are also required to complete the Annual Immunization Status Report indicating vaccination rates for the school district.
