This brief presents the Community Health Improvement Matrix, which local health departments can use to evaluate how a community health improvement plan or community health needs assessment and its related implementation plan addresses social determinants of health.
The authors introduce a new prevention type (primordial) to the traditional set of primary, secondary, and tertiary prevention, in order to better classify activities related to the social conditions that affect heath.
When to use
You could use the Community Health Improvement Matrix with community partners when discussing how public health professionals work on the social determinants of health. Community partners could complete this together as an activity, or public health employees could complete it and then shared with the group for discussion. The completed matrix would clearly indicate if and how the community's planning documents include the social determinants of health.
