Construction Plan Submittal Process for Healthcare Facilities
The Engineering Services team does final plan reviews for construction of new spaces and modifications to existing spaces. The request for Engineering Services to conduct a final plan review for a new (never licensed) health care facility requires that a license application and fee be submitted to Licensing and Certification at MDH. Once the licensing application has been processed by Licensing and Certification, a health facility identification (HFID) number will be assigned. The final plan review process cannot, under any circumstance, commence without the HFID number.

Instructions
A complete Construction Plan Submittal Package consists of the following components:
- A completed Construction Plan Submittal Form for Healthcare Facilities (PDF).
- One writable PDF copy of the certified (in accordance with MN. Rule 1800.4200) final construction plans. Digital specifications are not needed.
- A check payable to the "Commissioner of Finance, Treasury Division" in the amount specified by MN Statute 144.554: Health Facilities Construction Plan Submittal and Fees.
- We do not accept cash or credit cards.
- Fees are not refundable, per MN Statute 144.554.
- Checks must be accompanied by a copy of the Submittal Form.
An incomplete Construction Plan Submittal Package will result in a delay of the review and approval process.
Individual construction projects at the same healthcare facility require a complete and separate Construction Plan Submittal Package for each project.
- Send the completed Construction Plan Submittal Form for Healthcare Facilities (PDF) and the writable PDF copy of the plans to health.healthcareengineers@state.mn.us. We can accept large files by FTP if needed.
- Mail the check and a copy of the submittal form to:

Face-to-Face Meeting
If the construction plan will be reviewed in a face-to-face meeting with staff from the Engineering Services Section, the complete Construction Plan Submittal Package must be submitted to Engineering Services at least one (1) week prior to scheduling the meeting. A complete Construction Plan Submittal Package consists of the components listed above.
Construction Plan Submittal Package Exceptions
Some End Stage Renal Disease (ESRD) facilities will not require a final construction plan review by MDH, as the result of guidance received from the Centers of Medicare and Medicaid Services (CMS). Please review the CMS memo S&C: 13-47-LSC/ESRD, which includes the Life Safety Code Attestation for exempt ESRD facilities. However, it is recommended that you contact Engineering Services prior to submittal of any plans associated with an ESRD, and they will assist in determining if final plans need to be submitted.
For facilities designated as standalone* building B-Occupancy (no anesthesia, no patients incapable of self-preservation, and no overnight stays), Engineering Services would not be required to review the final plans. If you are unsure whether your B-Occupancy only facility needs a plan review, contact Engineering Services.
*NOTE: Facilities designated as a B-Occupancy that are connected to a Health Care Occupancy will require a plan review.
