Find My Immunization Record
The Minnesota Immunization Information Connection (MIIC) combines a person's immunizations into a single record, even if they were given by different health care providers in the state. This page includes information and resources for how to access and/or update your or your family's MIIC immunization record.
If you are looking for immunizations before 2002 or have lived in a state other than Minnesota, that information may not be available. Although most health care facilities submit immunization information to MIIC, not all facilities do. How to access MIIC immunization records
You can access an immunization record for you or a person whose record you have the legal authority to access in one of three ways; Use Docket to download and print your own PDF copy of a record, request a copy of a record by submitting a public request to MIIC, or have your local public health, primary health care provider, or pharmacy access MIIC and provide you with a copy of your record. Easily access immunization records for yourself, or a person whose record you have the legal authority to access through Docket. Docket gives people with a MIIC record a digital option to securely access their immunization history in MIIC. With Docket, users can:
- View your or your family's immunization history from MIIC.
- Check what vaccines you or your family may be due for.
- Review what vaccines you or your family may need in the future.
- Download and share a PDF copy of MIIC immunization records for health, school, travel, and other purposes.

Request a PDF version of your or your family's MIIC immunization records. To submit a request for you or a person whose record you have the legal authority to access, complete a MIIC Public Inquiry Form request. Requests will be processed within 14 business days in the order they are received.
Request a MIIC record update
If you or your family have a MIIC immunization record that is missing information or needs to be updated, you can submit a request for the MIIC record to be updated. You can only submit a request for yourself or a person whose record you have the legal authority to access. You cannot submit a request for your spouse/partner/adult child unless you have the legal authority to do so.
You can request to update the following information:
- Update demographics (name, gender, address, contact information, etc.) of a MIIC immunization record. This is the option to update demographic information to access a MIIC record using Docket.
- Add immunization information to a MIIC immunization record.
- Update privacy setting of a MIIC immunization record. This is the option to opt back in to MIIC, limit access to a MIIC record or opt out of MIIC.
- Receive a copy of a MIIC immunization record.
To submit a request for you or the person whose record you have the legal authority to access, complete a MIIC Public Inquiry Form request. Requests will be processed within 14 business days in the order they are received.
