Family Home Visiting Toolkits

These toolkits have been created for the purpose of spreading improvements and promoting best practices among programs delivering home visiting services throughout Minnesota. They have been informed by industry standards, evidence-based home visiting models, Health Resources and Services Administration (HRSA) requirements, and MDH quality improvement projects. Home visiting agencies can use these toolkits to assess and improve specific topic areas within their program.

Family Enrollment, Engagement, and Retention Toolkit

In 2018, MDH facilitated an eight-month statewide CQI learning collaborative on this topic. This toolkit was developed following the collaborative and includes information on changes tested, resources, and best practices for improving family engagement in family home visiting.

MDH participated in the HV CoIIN 2.0 Intimate Partner Violence Collaborative.
