Funding and Grants Management
Family home visiting programs at state, local, and tribal levels are funded by a variety of sources that work together to cohesively meet the needs of families. MDH administers federal Temporary Assistance for Needy Families (TANF) funding, the state-funded Nurse Family Partnership (NFP) grant, the Promising Practices grant program, the Evidence-Based Home Visiting (EBHV) grant, and the Maternal, Infant and Early Childhood Home Visiting (MIECHV) Formula and Expansion grants funded under the federal Affordable Care Act. As of 2023, the NFP, EBHV, and MIECHV grants are administered as one grant program, Strong Foundations: Evidence-based Family Home Visiting.

Strong Foundations: Evidence-Based Home Visiting Professional Development Funding
The Minnesota Department of Health (MDH) family home visiting section seeks applications from current Strong Foundations grantees interested in receiving additional funding to offer professional development opportunities for staff implementing evidence-based family home visiting models. Applications will be accepted on a rolling basis from Nov. 1, 2024—Oct. 31, 2025, for project activities occurring between Jan. 1—Dec. 31, 2025.

Promising Practices
Since 2023, MDH has funded tribal nations, non-profit organizations, and community health boards (CHBs) to expand access to community-based family home visiting programs for priority and hard to reach populations through the Promising Practices grant program. Promising Practices programs are needed to explore flexible and non-model approaches to family home visiting program implementation and service delivery. Priority populations include families experiencing housing insecurity or homelessness, substance use disorder (SUD), serious persistent mental illness (SPMI), and/or intimate partner violence (IPV); families currently living in a domestic violence shelter; and/or Black, Indigenous, and families of color with limited access to evidence-based or other family home visiting services.

Temporary Assistance for Needy Families (TANF) Family Home Visiting
Since 2001, MDH has administered family home visiting services for families at or below 200 percent of poverty and who are at risk for poor maternal and child outcomes. The Minnesota Legislature provides funding of $8,086,000 annually to all local public health departments and tribal nations for services provided under the TANF statute.

Strong Foundations: Evidence-based Family Home Visiting
Since 2023, MDH has funded evidence-based home visiting services in county governments, tribal nations, and non-profit organizations through the Strong Foundations grant program. Strong Foundations stabilizes and sustains existing home visiting programs while supporting start-up implementation of new home visiting programs seeking to address some of Minnesota’s greatest disparities over a five-year period.

Maternal, Infant and Early Childhood Home Visiting (MIECHV) federal grant
The federally funded MIECHV grant targets high-risk families who are most likely to benefit from intensive home visiting services. Awardees are selected based on designation as an at-risk community in a statewide needs assessment.
Nurse-Family Partnership (NFP) state grant
The state-funded NFP grant provides funds to local public health agencies and tribal nations to create or expand Nurse-Family Partnership programs.
Evidence-based Home Visiting (EBHV) state grant
The state-funded EBHV grant provides funds to local public health agencies, tribal nations, and non-profit organizations to implement evidence-based home visiting programs.
