Instructions for Local Public Health Act Annual Reporting

  • Finance and Staffing Instructions for 2024 (PDF): These measures track workforce/staffing and expenditures/finance over time across the local public health system in Minnesota. Review these instructions before entering data into REDCap. Please share these instructions with everyone on the community health board who has a role in annual reporting on Local Public Health Act finance and staffing.
  • Performance Measurement Instructions for 2024 (PDF): These instructions will help you enter performance measures data into the REDCap reporting system for Local Public Health Act (LPH Act) annual reporting for calendar year 2024. Reporting for 2024 will open mid-February 2025 and must be completed by March 31, 2025.
  • Performance-related Accountability Requirement Instructions for 2024 (PDF): In December 2023, the Commissioner of Health, in consultation with the State Community Health Services Advisory Committee (SCHSAC), determined for calendar year 2024, community health boards must report on their ability to meet Measure 1.3.3 Use data to recommend and inform public health actions.
  • Validation: If you are a CHS administrator, remember to complete validation to indicate that staff have finished entering all data.
