Lead Licensing

Applications 

  • Individual Lead License - Apply Online for new applicants.
    To apply for a worker, supervisor, inspector, risk assessor, or project designer license.
  • Lead Firm Certification - Apply Online for new applicants.
    To apply for a lead firm certification.
  • Lead Training Course Permit - Apply Online.
    To apply for a training course permit at least 60 calendar days in advance of holding the course.

Note: This is not for renewals. Please contact us if you need help on how to renew.

Notifications

  • Lead Training Course Notification - Apply Online.
    To meet the requirement for training course providers to notify MDH five calendar days before the start of a training course.
    • Training Course Notice - Student Roster
      Fill out the name and diploma number of each student that attended and passed your training course.
  • Lead Hazard Reduction Notification - Apply Online.
    To notify MDH of lead hazard reduction work at least five calendar days before the project begins. Use this system for initial notifications, amendments, and cancellations.

Variance Request for Asbestos or Lead - Apply Online.
To request a variance of a Lead Hazard Reduction Notification or a Lead Training Course Notification.

Template

Lead Hazard Reduction Report Template (PDF)
Use this template for lead hazard reduction reports. Upon completion of regulated lead work, certified lead firms must complete and maintain this report for three years.
